Qualifications
- This role will primarily offer consultation & education for people living with diabetes
- A minimum of three (3) years of experience in a community or public health setting.
- In-depth knowledge in the following areas: methods for assessing health status, current theories and practices, health systems in general and community health in particular, knowledge of primary care, determinants of health.
- Registered Dietitians Required
- Member in good standing of the College of Dietitians of Ontario
- Must speak and write in French
- Preferably, a certificate of membership in the Canadian Dietitian Association
- Must have proof of COVID vaccine, liability insurance, TB testing and vulnerable sector police check
- Membership in Dietitians of Canada – Gerontology Network an asset.
- Ability to work in a team and independently
- Good verbal and written communication skills
- Excellent organizational and time management skills
- Demonstrates an ability to problem solve
- Computer Literacy - knowledge of computerized electronic medical records (EMR) programs an asset
- Travel may be required with some positions
Responsibilities
- Conduct assessments and determine, in consultation with clients, the appropriate intervention.
- Identify individual needs and common problems.
- Plan and develop services and programs according to identified needs.
- To offer and deliver appropriate services and programs on an individual or group basis.
- Support clients and the community in their efforts to obtain appropriate nutrition, health and community support services.
- Organize the necessary referrals in consultation with the clientele and the interdisciplinary team and ensure the continuity of follow-ups.
- Collaborate and liaise with clients, community, staff, consultants, community groups and other appropriate agencies to address identified needs.
- Maintain complete and accurate computerized records.
- Participate in the evaluation of services and programs and offer suggestions accordingly.
- Actively participate in prevention, education, health promotion, awareness and community development of clients and the community, in collaboration with the interdisciplinary team.
- Perform all other related duties.
