Seasons Care Dietitian Network is a consulting network of Dietitians and Nutrition Managers specializing in Long-Term Care and Retirement Homes. Since 1996, Seasons Care has become the leader in Seniors Nutrition in Canada. We partner with many Long-Term Care and Retirement Homes by providing consulting Dietitian and Nutrition Management services to support them in making a difference in the lives of their Residents. Qualifications

  • This role will primarily offer consultation & education for people living with diabetes
  • A minimum of three (3) years of experience in a community or public health setting.
  • In-depth knowledge in the following areas: methods for assessing health status, current theories and practices, health systems in general and community health in particular, knowledge of primary care, determinants of health.
  • Registered Dietitians Required
  • Member in good standing of the College of Dietitians of Ontario
  • Must speak and write in French
  • Preferably, a certificate of membership in the Canadian Dietitian Association
  • Must have proof of COVID vaccine, liability insurance, TB testing and vulnerable sector police check
  • Membership in Dietitians of Canada – Gerontology Network an asset.
  • Ability to work in a team and independently
  • Good verbal and written communication skills
  • Excellent organizational and time management skills
  • Demonstrates an ability to problem solve
  • Computer Literacy - knowledge of computerized electronic medical records (EMR) programs an asset
  • Travel may be required with some positions

Responsibilities

  • Conduct assessments and determine, in consultation with clients, the appropriate intervention.
  • Identify individual needs and common problems.
  • Plan and develop services and programs according to identified needs.
  • To offer and deliver appropriate services and programs on an individual or group basis.
  • Support clients and the community in their efforts to obtain appropriate nutrition, health and community support services.
  • Organize the necessary referrals in consultation with the clientele and the interdisciplinary team and ensure the continuity of follow-ups.
  • Collaborate and liaise with clients, community, staff, consultants, community groups and other appropriate agencies to address identified needs.
  • Maintain complete and accurate computerized records.
  • Participate in the evaluation of services and programs and offer suggestions accordingly.
  • Actively participate in prevention, education, health promotion, awareness and community development of clients and the community, in collaboration with the interdisciplinary team.
  •  Perform all other related duties.
