Qualifications

  • Registered and in good standing with the College of Dietitians of Ontario
  • Must provide proof of:
  • Liability insurance
  • TB testing
  • Vulnerable sector check
  • Ability to work both independently and within a multidisciplinary team
  • Strong written and verbal communication skills
  • Excellent organizational and time management abilities
  • Demonstrated problem-solving skills
  • Proficiency with computers, including experience using electronic medical records (EMR)
  • Travel may be required for certain positions

Responsibilities

  • Identify and monitor the level of nutrition and hydration risk for all Residents
  • Complete scheduled RAI assessments: on admission, quarterly and significant status changes
  • Prioritize and respond to dietary referrals in a timely manner
  • Develop, evaluate, and modify individualized nutrition care plans
  • Liaise with Residents, families/SDMs, and the team to implement effective dietary interventions
  • Conduct swallowing assessments as required
  • Provide nutrition counselling and education to Residents and their families
  • Collaborate with the Nutrition Manager to enhance the dining experience
  • Participate in Care Conferences and committee meetings as per organizational needs
  • Review and approve seasonal menus, and create individualized menus where applicable
  • Provide training and support to dietary and nursing team
  • Contribute to continuous quality improvement (CQI) initiatives
  • Ensure all work complies with Ontario Regulation 246/22, Section 80, under the Fixing Long-Term Care Act, 2021
  • Perform additional duties as required
